655 VALUE OF CEREBROSPINAL FLUID TUMOR NECROSIS FACTOR-ALPHA (TNF-ALPHA) FOR RAPID DIAGNOSIS OF BACTERIAL MENINGITIS By IMAN FAWZY MONTASSER1, EMAN MOHAMED EL-GINDY1, ENAS HASSAN ALLAM1, HOSAM S. ELBAZ2, REHAM ALI AHMED KHALIFA3 AND HANY EL SAYED ALI Departments of Tropical Medicine1, Internal Medicine2, Microbiology and Immunology3, Faculty of Medicine, Ain Shams University, Cairo 11566 and Embaba Fevers Hospital, Giza, Egypt Abstract Meningitis is common in tropical areas and also in Egypt and has a world-wide distribution This study evaluated the potential role of CSF TNF α in diagnosis and differential diagnosis of acute meningitis (bacterial versus aseptic meningitis). This case-control study was conducted between Ain Shams University Tropical Medicine Department and Embaba Fever Hospital. Fifty patients with suspected meningitis were recruited during from January 2014 to June 2014. They were divided according to culture results into 2 groups: GI: 40 patients with acute bacterial meningitis (proved by CSF culture), G2: 10 patients matched according to age and sex with clinical sings of CNS infection but without laboratory evidence of bacterial origin, (Suspected cases, and negative culture). Both groups were subjected to thorough history taking, full clinical examination, and laboratory investigations including CSF analysis & CSF TNF was measured by ELISA. The results showed a highly significant difference between cases and control regarding CSF TNF (P=0.00). The criteria’s of diagnostic validity test was 100% for all at cutoff ≥275ng/ml and ≤700ng/ml with 100% specificity and sensitivity. A significant correlation between CSF-TNF and each of ESR (P=0.003) & CSF cells (P=0.015), without significant correlation regarding other parameters (P>0.05).

Serum procalcitonin and cerebrospinal fluid cytokines level in children with meningitis.
AIMS To determine the level of serum procalcitonin and cerebrospinal fluid cytokines in children with bacterial or viral meningitis and to document the use of these parameters in differential diagnosis. RESULTS Before the start of antibiotic treatment, serum procalcitonin and tumor necrosis factor alpha levels were found to be higher in acute bacterial meningitis compared with viral meningiti...

To evaluate the potential role of cachectin/TNF-alpha in the pathogenesis of bacterial and viral meningitis, concentrations and kinetics of TNF-alpha were determined in cerebrospinal fluid (CSF). After intracerebral, but not systemic, infection with Listeria monocytogenes in mice, TNF-alpha was detected as early as 3 h after infection reaching maximum titers after 24 h. However, TNF-alpha was n...
متن کاملIntrathecal production of interleukin-12 and gamma interferon in patients with bacterial meningitis.
To assess the role of interleukin-12 (IL-12) and gamma interferon (IFN-gamma) in children with bacterial meningitis, bioactive IL-12 (p70) and the inactive subunit p40 and IFN-gamma were measured in serum and cerebrospinal fluid (CSF) from 35 children with bacterial meningitis and 10 control subjects. The production of IFN-gamma is induced by IL-12 with tumor necrosis factor alpha (TNF-alpha) a...
متن کاملTransforming growth factor beta 2 inhibits cerebrovascular changes and brain edema formation in the tumor necrosis factor alpha-independent early phase of experimental pneumococcal meningitis
Macrophages and granulocytes seem to play a key role in the pathogenesis of bacterial meningitis. Transforming growth factor beta (TGF-beta) leads to macrophage deactivation, as well as to inhibition of cytokine production and of endothelial granulocyte adhesion. We have investigated the influence of TGF-beta on regional cerebral blood flow (rCBF), intracranial pressure (ICP), and brain edema f...
متن کاملTyrosine kinase inhibition reduces inflammation in the acute stage of experimental pneumococcal meningitis.
Bacterial meningitis is an acute inflammatory disease of the central nervous system with a mortality rate of up to 30%. Excessive stimulation of the host immune system by bacterial surface components contributes to this devastating outcome. In vitro studies have shown that protein tyrosine kinase inhibitors are highly effective in preventing the release of proinflammatory cytokines induced by p...
